What requirements do I need to bring when I want to process my SimplySmile Membership Plan?
Requirements:
1. 1 Valid ID (including beneficiaries if availed)
2. Completing your Membership Form
3. Your Payment for your respective Plan
Can someone else redeem my purchased SimplySmile Membership Plan?
Yes, your registered beneficiaries. You also have the option to purchase a SimplySmile Membership Plan for someone else but, only the person (including the beneficiaries) whose name appears on the Membership Card can redeem the benefits.
Where can I redeem my purchased SimplySmile Membership Plan?
Your purchased SimplySmile Membership Plan can be redeemed in the Clinic at Door 2, Liceo Arcade, Liceo de Cagayan University, R.N. Pelaez Boulevard, Cagayan de Oro.
What are the modes of Payment?
You can either pay in Cash, Credit or Debit Card, or through Installment Plans. Here are the details:
- CASH: Pay in full during your visit
- CREDIT / DEBIT CARD: Pay during your visit through POS or here on our website through Paypal.

Age Eligibility
– Principals: 18 to 65 years old
– Beneficiary: 6 to 55 years old
Completion of Application Form is required
Membership fees are paid based on a renewal basis only
Lifetime Discounts: All indicated treatments
Standard Whitening is for one session only. For Premium Whitening (2 sessions), conditions are based on the Lifetime Discount.
Cash back on Xrays, will be given only through purchased Membership RIDERS:
- by having a prescription from our clinic to the respective dental laboratories
- by sending us an Official Receipt
- through Fund TransfersEither the Principal and / or Beneficiaries can consume every treatment under the plan until the expiry date or until fully depleted
All benefits are subject to Zahn Aesthetix Dental Clinic’s General Exclusions and Conditions for eligible accounts
